• Location: Larimer County, Fort Collins Location
• Status: Full Time, Salary - Position provides same day and scheduled point-of-entry services including integrated assessments, treatment recommendations, diagnosing, screening, referral, crisis intervention, and case management to all ages seeking mental health and/or substance use disorder treatment. Position works closely with other clinical teams to ensure appropriate placement of new clients in the continuum of SummitStone services. Provider collaborates with community and healthcare providers to ensure continuity of care. At times may provide psychoeducation, and program orientation to prospective clients. This clinician will enrich the team's diverse experience by broadening the range of ways of understanding and engaging with our communities through culturally responsive service to others. - Essential Duties:
  • Conduct initial integrated clinical assessments
  • Direct client care to include care coordination, behavioral health related screenings and assessments, case management, -and related clinical services.
  • Serves as a clinical resource person to building site(s), clinical staff, case managers, medical providers and collaterals.
  • Knowledge of professional policies and procedures.
  • Collaborates with other staff in providing, planning, developing, implementing, and evaluating clinical services, including collaborating with Clinical Managers and Program Supervisors.
  • Provides on going care coordination within SummitStone, community agencies and resources. -
  • Provides crisis intervention to include crisis assessment and de-escalation.
  • Concurrent clinical documentation in Electronic Medical Record system -
  • Supports and fosters team values with peers and clinic staff, to provide support and remove barriers.
  • Maintains superior customer service for internal and external customers by providing information and answering questions/requests in timely matter.
  • May participate on committees (both internal and external), in meetings, and works on projects as assigned.
  • Perform other duties as assigned. - NOTE : Be aware that licensure must be pursed in this role and have a willingness to engage on the LAC track. - - Required: -
    • Master's degree in social work, counseling, psychology, or related field from an accredited College or University and - registered with DORA as a Candidate working towards licensure as LPC, LCSW, or LMFT. -
    • At least one year of experience in the behavioral health field.
    • At least one year of experience with mild to severe mental illness and substance use disorders. -
    • Bilingual (English/Spanish) and able to pass the -linguistic proficiency -assessment - Preferred: -

          • Receive supervision toward clinical licensure (i.e. LCSW, LPC, LMFT, LAC) and/or certification (i.e. CAS) at no cost to the employee.

            • Master's Level Unlicensed: $58,011 - $69,618 annually
            • Master's Level Licensed: $66,040 - $79,217 annually
