Job Description:
A company is looking for an RN Case Manager.
Key Responsibilities
Coordinate all aspects of a patient's care and serve as a patient
advocate
Conduct needs assessments and develop care plans in collaboration
with the care team
Support patients during care transitions and ensure ongoing care
coordination
Required Qualifications
Current RN License in good standing in the state of practice
Bachelor's degree in nursing or an associate's in nursing with
another bachelor's degree
Minimum of 4 years of clinical experience, with at least 2 years in
case management
Certified Case Manager (CCM) certification or commitment to
obtain
Geriatric care experience is highly desired
