Job Description:
Join our client's dynamic team as a Licensed Clinical Social
Worker (LCSW) at the forefront of primary care treatment! In this
role, you will be instrumental in identifying, triaging, and
managing patients with medical and behavioral health conditions
within our cutting-edge Rural Health Clinic. Utilizing
psycho-education strategies, you will provide skill training and
interventions tailored to the goals set in partnership with each
patient. Your expertise in prevention, assessment, and treatment of
mental, emotional, behavioral, and addictive disorders will be
crucial as you engage in multidisciplinary treatment planning and
clinical social work interventions, including patient and family
psychotherapy. Collaborating closely with the care management team,
you will provide direction and input for patients enrolled in
Behavioral Health Integration, while also working alongside
community health workers to address unmet social needs and engage
community members in care and supportive services through Community
Health Integration. We are seeking a flexible and adaptable LCSW
who thrives in a fast-paced, nontraditional clinical environment,
utilizing evidence-based and culturally sensitive best practices to
make a meaningful impact on the lives of our patients.

ESSENTIAL DUTIES & RESPONSIBILITIES:
Interview and assess patients and assist throughout patient care
plan. Screening, assessment, and treatment for behavioral health
conditions (e.g., depression, anxiety, substance use)
Use of common screening tools and best practices (e.g., PHQ-9,
GAD-7, SBIRT, etc.) Contributing to team-based care for the
management of chronic health conditions (e.g., diabetes,
hypertension, heart disease, obesity, etc.)
Assist in the detection of "at risk" patients and develop of plans
to prevent further health complications Perform Social Determinant
of Health Risk Assessments; Psychiatric Diagnostic Evaluations;
Individual and Family Psychotherapy Services
Demonstrated ability to collaborate on a multi-disciplinary team
Working knowledge of evidence-based psychosocial treatments for
common mental health and substance use disorders
Document in Electronic Health Records in a timely, accurate, and
clinically sound way, and communicates critical information to the
medical team Monitor health outcomes at an individual and
population level Identifies and provides helpful resources to
patients and families.
Able to adapt services to language and cultural preferences; and
promote diversity among the providers working in teams.

Remains knowledgeable on current federal, state and local laws,
accreditation standards or regulatory agency requirements that
apply to the assigned area of responsibility and ensures compliance
with all such laws, regulations and standards.
Ability to operate various software applications including by not
limited to electronic medical records, care coordination platforms
and video technology.
Ability to assist and educate patients in the use of telehealth
equipment.
Ability to solve practical problems and deal with a variety of
concrete variables in situations where only limited standardization

QUALIFICATIONS:
Required
Master's degree in social work (MSW) from an accredited university
or institution
2 years clinical social work experience
LCSW license in the State of Wyoming - current and in good
standing
Preferred:
Prior work experience as a Social Worker in medical setting, FQHC
or RHC.
Prior work with diverse patient population
Prior work experience in integrated behavioral health models
