Job Description:
A company is looking for a Case Manager RN (RN in NV & CA).
Key Responsibilities
Facilitate the delivery of healthcare information and determine
eligibility for benefits
Develop and implement health strategies and programs to promote
member wellness
Coordinate care and advocate for services to meet comprehensive
health needs
Required Qualifications
Active and unrestricted RN licensure in CA and NV compact
Minimum 3+ years of clinical practical experience, preferably with
Medicare members
Minimum 2+ years of case management, discharge planning, or home
health care coordination experience
Associates Degree or Nursing Diploma required; Bachelor's Degree
preferred
